Blood Sugar Log Printable Template - Portrait

When my wife was pregnant with our son, she was diagnosed with gestational diabetes. To help her manage her meds and blood sugar, I created a handwritten log that tracks both. Since then I've made it into digital form, and you can now download it and print it for your purposes.

I've seen a lot of blood sugar log templates before but there were none that track time, meds, and blood sugar at the same time, so this design has gone through a number of iterations.
